What is Case Management?
Provides assistance to individuals who desire to gain access to services, authorize and coordinate services provided through the Minnesota Department of Human Services waivered services, monitor the delivery of services adjusting them to meet the individual’s continuing needs and advocate for the right of the individual to assure his or her health safety.
Personal care assistance services help a person with day-to-day activities in their home and community. PCAs help people with activities of daily living, health-related procedures and tasks, observation and redirection of behaviors and instrumental activities of daily living for adults. It is available to eligible people enrolled in a Minnesota Health Care Program. A PCA Assessment is done in the client’s home to determine eligibility and PCA time.

All individuals 6 months old and older are encouraged to get a flu shot. Please call our office to set up an appointment.
Norman-Mahnomen Public Health provides foot cares in the office by appointment. Contact our offices to set up an appointment. The cost is $25.
Adult and Child immunizations are available in the office by appointment. Those individuals who are eligible for Minnesota Vaccine for Children can receive an immunization for $5 per immunization. No one will be refused due to inability to pay.
