To obtain a United States Passport you will need to present the following:

1. Completed passport application form (you MUST sign in person) - obtain from the County Auditor/Treasurer’s Office or by clicking on the link above to the Passport Agency's web site http://travel.state.gov/passport; and find the Passport Application. 

2. A certified Copy of Birth Certificate from the County or State level (Not a Hospital Certificate). 

3. One passport photograph that meets passport requirements. 

4.  Primary Identification (One of the following):

• Previously issued, undamaged U.S. passport
• Naturalization Certificate
• Valid Driver's License
• Current Government ID (city, state or federal)
• Current military ID (military and dependents)

Minors 15 and under must appear with both parents and parents must present valid Driver's Licenses.  Minors 16 and older, if they have a valid Driver's License, may appear without a parent. If a minor 16 years old or older has no I.D., one parent must appear with the minor. 

5. Fees:

$35.00 cash or check to Norman County Treasurer

$100.00 check to US Dept of State

Passport Card
Adult: (16 & over)

6. If leaving in less than 4 weeks, you may expedite the passport application by paying a $60.00 expedite fee per passport. The fee is in addition to the cost of the passport and there may also be express mail fees. 

7.  If one parent cannot appear with the child to sign, we must have a signed and notarized statement from that parent. A form can be found under the Passport Agency's website. 

Processing time varies. Due to high demand, it is now taking 6-8 weeks until you will receive your passport in the mail.
